“Each morning when I open my eyes I say to myself; I, not events, have the power to make me happy or unhappy today.  I can choose which it shall be.  Yesterday is dead, tomorrow hasn’t arrived yet.  I have just one day, and I’m going to be happy in it.” -- Grouch Marx

            Guest Speaker:
                          Bob Ostroff MD

bob Spokesperson for Southern Sonoma Medical Group which represents physician interests in Petaluma especially Petaluma Valley Hospital, which is leased to St. Joseph Hospitals.  SSMG organizes MDs  into a coherent group to help with physican recruitment and retention issues and to provide adequate patient services for this area,   Due to this areas poor livability index (ie high cost living and rural type wages), our area has trouble recruiting doctors (unlike the more urban SF bay/Marin areas).  Potential recruits like our area but contracts limit affordability in housing, wages, reimbursements, etc. Currently SSMG is contracting with MARIN IPA to extended the medical services network.

Joel Christie
Represents Marin IPA, an organization of about 300 physicans.  Marin IPA  primarily contracts with the six large HMOs for services.  Helps compete vs the Kaiser Permante HMO (which has about 40% of insured patients).  Joel wants businesses to consider Marin IPA for their employees.  Petaluma Kaiser patients don’t have an emergency room or local cardiac services.
jelQ and A......How does SSMG work?  After Health Net had problems, most MDs pulled out.  SSMG offers income guarantees, financing and setup help with 3-4 yrs recruitment incentives.

What does Wayne Fairchild departure (from PVH recently) affect SSMG?   SSMG was understandably distressed and distraught with Wayne’s sudden departure since they had a good working relationship and shared a common vision for health care delivery in Petaluma.  At this time, St. Josephs may yet let SSMG be a part of recruitment process for his replacement.

How do zipcodes affect reimbursements?  Medicare fees schedules are based on zipcodes- Medicare payments in Sonoma County are rated for rural reimbursements (Marin County is not consider rural).  Although MARIN IPA does not use zipcodes in its reimbursement schedule.

How do Marin IPA premiums compare versus Kaiser ones?.  Hard to say since it depends on who underwrites the insurance plan and what benefits are defined, ie. is there a drug benefit, etc.   Rotarians also mentioned that you should factor in the cost in time and money of employee transporation to the Kaiser facilities for non-local services like the emergency room when making comparisons.

Good questions guys!  That’s all for now. Ciao!
